Our office is on the ground floor with plenty of parking. Knock on door, suite 200, to gain entrance.

Instructions:

Once we receive notification that you are interested in volunteering, we will send you our Choice volunteer application bundle. Please fill it out and return it to us with 2 forms of current ID. The background check goes quickly once this is turned in. All medical facilities require background checks now due to fragile patient information in our offices.
