Administrative Volunteer- Tuck In Calls

Purpose

Tuck-In Call Volunteers provide compassionate support to hospice patients and their caregivers through a brief, friendly phone call before the weekend or other designated period. The goal is to help ensure patients and families have the supplies, medications, information, and support they need while providing reassurance and a caring connection.

Responsibilities

• Make scheduled “tuck-in” calls to hospice patients and/or caregivers.
• Follow an established call script or checklist provided by the hospice team.
• Ask whether the patient has enough medications, medical supplies, food, and other essential items needed through the weekend.
• Confirm that the family knows how to reach the hospice team if a need or concern arises.
• Listen attentively to patients and caregivers and provide emotional support through conversation.
• Identify and report concerns, changes, or requests to the appropriate hospice staff member.
• Document completed calls and any concerns according to hospice procedures.
• Maintain patient confidentiality and follow HIPAA and hospice privacy requirements.
• Communicate with the Volunteer Coordinator regarding call completion, concerns, and follow-up needs.
