    About Human Health Project (HHP)

    Human Health Project is a global holistic health literacy charity building a Unified Patient Experience (UPE)—a platform and service model that helps patients navigate healthcare, improve health literacy, and access care, education, information and advocacy.

    We are now launching a reimbursable care delivery model under Medi-Cal (CalAIM), Medicaid and Medicare, leveraging a trained volunteer workforce.

    HHP is building a national model for Community Health Worker (CHW)-driven care delivery, focused on:

    • Health literacy
    • Care navigation
    • Social determinants of health

    The Role

    We are seeking a Care Delivery & Workforce Lead to operationalize HHP’s volunteer workforce into a structured, high-quality care delivery system.

    Key Responsibilities

    • Design and manage CHW workflows:
      • Patient navigation
      • Medicare / Medi-Cal support
      • SDOH referrals
    • Build and manage:
      • Volunteer tiers (Tier 1 / Tier 2)
      • Care delivery teams
    • Develop training programs:
      • CHW readiness
      • Documentation standards
    • Assign volunteers to pilot patient cohorts

    Ideal Background

    • Healthcare operations or care coordination
    • Volunteer or workforce management
    • Public health or social services experience
