Description
The Care Transition program has been established to ease the burden of our community members who lack adequate family or caregiver resources and transportation assistance when transitioning from long-term care facilities back to their home and through recovery.
Volunteers will be matched with a client for a three-month or 60 hour period to offer:
- Companionship
- Respite
- In-home support, such as light homemaker services and fall prevention
- Transportation to non-emergency medical or pharmacy appointments
- Assist staff with in-home client assessments
- Transportation home from the long-term care facitlity
Volunteers have a right to say "no" to any services that is requested. Staff will work with clients when volunteers wish to not assist with certain services.
Volunteer in the program receive:
- A stipend of up to $500 reimbursed at a hourly rate for services completed
- Passenger Mileage reimbursement at $0.70 per mile
- Meal reimbursement for both themselves and the client
