    Description

    Compassionate Care Hospice is looking for volunteers to meet with our patients who are on hospice and collaborate with their families to collects photos/mementos to create a legacy-type board or a collage. You will ask the patient and/or family a series of questions based on getting to know the patient. With all the information you accumulate. the fun part begins! You will use your creative mind to create a collage of pictures, cut outs, words, etc..

    This is for the dementia patients to have in their rooms to help remember, the care team can see all the interesting things that the patient may no longer be able to tell them. It makes connections for everyone and becomes a treasured keep sake.
