    Responsibilities:

    • Provide resource referrals and psychosocial support to patients and families
    • Support intake and needs assessments
    • Collaborate with Patient Navigators and healthcare partners
    • Maintain ethical and professional standards of care

    Preferred Qualifications:

    • Licensed Social Worker (LCSW, LMSW preferred)
    • Experience in healthcare, oncology, or community-based services a plus
