Preferred Peaceful Transitions Hospice

We at Preferred Transitions have made it our mission to emphasize patient care above all else. Preferred Transitions accepted our first home health patient in July 2016 and cared for 65 patients during our first year. In 2017, we grew to provide care to over 400 patients and in 2018 we exceeded 800 patients. We have been growing ever since and believe the reason for this growth is our philosophy for patient care and staff satisfaction. This is illustrated by our continued Medicare Patient Satisfaction Survey Rating of 4-5 Stars. We accepted our first hospice patient in May of 2024 and are excited to continue growing the program by adding services such as pet therapy, music therapy and alternative therapies.
