Community-Based Clinical Counselor

The Community-Based Counselor – Substance Use Disorder (SUD) Services provides comprehensive, community-based counseling and case management services to individuals experiencing substance use disorders and co-occurring mental health conditions. Working in community settings, schools, and other natural environments, the counselor conducts clinical assessments, develops individualized treatment plans, delivers evidence-based individual, group, and family counseling, and supports clients throughout the recovery process. The counselor utilizes motivational interviewing, relapse prevention strategies, and trauma-informed care to promote recovery, improve functioning, and reduce barriers to treatment. This role collaborates with healthcare providers, community agencies, families, and recovery support systems to coordinate care, connect clients with resources, and ensure continuity of services. The Community-Based Counselor maintains timely and accurate clinical documentation, participates in crisis intervention when needed, and adheres to ethical, regulatory, and organizational standards while fostering recovery, resilience, and long-term wellness.

Minimum qualifications are licensure and master's degree in counseling related field with 2 years experience. Certification as an addiction counselor is required within 3 years of hire.
