ECM Case Manager

AACI is looking for an ECM Case Manager with experience supporting multiple Populations of Focus (POFs) and coordinating care for members with complex medical, behavioral health, housing, and social needs.

We're especially interested in candidates whose experience extends across different high-risk populations rather than being limited to one area of case management.

Your experience may include working with populations experiencing:

• Complex medical and behavioral health needs

• Housing instability or homelessness

• Frequent healthcare or emergency-service utilization

• Significant barriers to accessing ongoing healthcare

• Multiple social determinants impacting their health and stability

Strong candidates will also have experience with medical/clinical notes, care plans, assessments, and maintaining detailed clinical documentation.

In this position, you'll conduct outreach, enroll eligible Medi-Cal members into ECM, complete assessments, develop individualized care plans, coordinate with multidisciplinary clinical teams, and connect members to healthcare and community resources.

We're looking for:

✓ 1+ year of case management or related experience

✓ Experience with multiple populations of focus

✓ Medical/clinical documentation experience

✓ Care coordination experience

✓ Familiarity with housing and community resources

✓ Ability to work effectively with multidisciplinary teams
