CHLMS MEDI-HELPZ FOUNDATION

a. To establish and provide Patient Education programs. The goal of these programs

will be to improve the health literacy of all patients, targeting the underserved

communities.

b. To create Patient Empowerment programs and to increase patient knowledge of

available healthcare resources. These available resources will be useful in ensuring

the best health outcomes on a personal, family, and community level.

c. To develop Patient Engagement initiatives to encourage collaboration with external

community organizations to remove barriers to optimal healthcare access for

underserved communities.

d. To host virtual and in-person patient webinars and seminars, as well as to

participate in events and activities that promote Patient Education, Patient

Empowerment, and Patient Engagement.
