Our mission is to design, develop and implement strategies to decrease the incidence of cardiac disease, stroke, and related chronic disease in Northern New York State.

All our activities are aimed at reducing tobacco use and increasing physical activity and nutrition – the top 3 preventable causes of chronic disease that often disproportionately impact people living in rural communities
