Community Health Trust of Pajaro Valley

CHT has focused on health equity in the Pajaro Valley for over 25 years. CHT’s Diabetes Health Center was established in 1998 in response to an overwhelming number of patients seeking emergency care at Watsonville Community Hospital for uncontrolled diabetes.

Today, we serve as a provider, funder, and partner bettering people's lives through several initiatives. As a Provider, we expand equitable access to wellness services through nutrition education, chronic disease management, access to fresh food, and fitness classes. As a Funder, we invest in health equity and wellness through grants, scholarships, and endowments, helping local efforts thrive. As a Partner, we collaborate across sectors to connect people, resources, and ideas, advancing health and wellness and addressing community needs.
