Partnerships for Advancing Community Health (PACH) is a statewide nonprofit working at the intersection of community voice, health equity, and systems change. We convene and support Ohio’s certified Community Health Networks—alongside community health workers (CHWs), local organizations, and partners—to strengthen how communities identify and address the social drivers of health. Our work is grounded in collaboration, trust, and practical innovation: we focus on building shared infrastructure, advancing sustainable funding models, and elevating the CHW workforce as essential leaders in community-based care. PACH serves communities across urban and rural Ohio, with a deep commitment to centering lived experience, reducing inequities, and ensuring that community-driven solutions are recognized, valued, and sustained within the broader health system.
