    Description

    Patient Advocate Volunteer

    The Volunteer Hospice Patient Advocate serves as a compassionate liaison between patients and the hospice care team. This role focuses on enhancing the patient's experience by conducting scheduled telephone follow-ups with patients receiving hospice services. The advocate ensures patients feel supported, heard, and informed throughout their care journey.

    Key Responsibilities

    • Conduct telephone follow-ups with patients at scheduled intervals to assess satisfaction and identify any questions or concerns regarding their hospice care.
    • Actively listen and provide empathetic, caring communication during all interactions.
    • Document patient feedback accurately and promptly in MatrixCare (EHR).
    • Communicate patient concerns and feedback to the hospice care team for timely resolution.
    • Maintain confidentiality and adhere to HIPAA regulations at all times.
    • Ensure timely follow-up and accurate reporting to support quality care delivery.

    • Strong, compassionate communication skills with the ability to build trust and rapport.
    • Excellent listening skills and attention to detail.
    • Ability to advocate for patients respectfully and effectively.
    • Demonstrated reliability in completing follow-ups and documentation on time.
    • Commitment to hospice philosophy and patient-centered care.
    • Understanding of HIPAA guidelines and practices
